Flu Clinic Forms Due

If your child is participating in Hill’s Flu Clinic, please return your 1) COMPLETED FLU FORM and 2) COPY OF YOUR INSURANCE, CHIP, OR MEDICAID CARD to the front office TOMORROW.  If you do not have an insurance, CHIP, or Medicaid card your child can still receive a flu shot with just the completed flu form.  If you do have medical coverage, though, it is imperative that we receive a copy of your card at the same time we receive your completed flu form.  Click here for additional information and forms.  Please contact Hanna at hanna.curran@austinisd.org with any questions and to submit any missing items.
